Welcome and Thank You for Choosing Us for Your Eyecare Needs


We are pleased that you have entrusted us with your vision care. Your time is important to us, and we strive to provide efficient, high-quality service during your visit.


Please note that appointments with a specialist can be time-intensive. Depending on your condition and the testing required, your visit may take up to two hours or more. 


We appreciate your patience as we work to ensure that all necessary diagnostics are completed and that you are seen by the provider as promptly as possible.

Appointment Policy

Appointments are scheduled to allow us to provide the most efficient and timely care to all our patients. If you are unable to keep your scheduled appointment, we kindly ask that you notify our office at least 24 hours in advance to cancel or reschedule.


We understand that unforeseen circumstances may occasionally prevent you from giving advance notice. While we appreciate as much notice as possible, we recognize that emergencies can occur and will do our best to accommodate such situations.


Thank you for your cooperation and understanding as we strive to provide the highest level of care to all our patients.

Insurance and Payment Policy

We are pleased to accept most commercial insurance plans as well as Medicaid.


As a courtesy to our patients, we make every effort to verify that your insurance is active and current using the resources available to us. However, due to the complexities and frequent changes in insurance plans and coverage, we cannot guarantee benefit eligibility or coverage.


Our team is committed to helping you maximize your insurance benefits and will gladly answer any questions you may have to the best of our ability.


Please note: Copayments are due at the time of your visit.


We value the opportunity to participate in your eyecare and appreciate your trust in our office.

What to Expect During Your First Visit

As part of your comprehensive initial eye exam, your eyes will be fully dilated to allow for a complete evaluation of your eye health. The effects of dilation may last 2 to 5 hours after your visit and may persist longer in individuals with light-colored eyes.


Following your exam, we will provide temporary post-dilation sunglasses for your comfort—unless you bring your own.


Please plan accordingly, especially if you are sensitive to light or unsure about driving after your appointment.

Important Information for Your Upcoming Visit

To help us serve you efficiently, please bring the following items with you to each visit:


  • Driver’s License or Photo ID
     
  • Current health insurance cards
     
  • Prescription glasses (if applicable)
     
  • A current list of all medications
     
  • Copay, which is required at the time of your visit
     

We also kindly ask that you complete and review the attached forms prior to your appointment. If you have any questions while filling them out, please don’t hesitate to contact us—we’re happy to help.


In addition, please be prepared to provide the following information:

  • The name of your primary care physician
     
  • The name of the referring provider or patient
     
  • Any other doctors who may need to receive reports regarding your care
     

If you are unable to keep your appointment, we ask that you notify our office at least 24 hours in advance. Please refer to our Late Cancellation and No-Show Policy for more details.


We strive to make your visit as comfortable and enjoyable as possible. If there is anything we can do to enhance your experience, please let us know.
